DISCLOSURE AND CONSENT TO DUAL AGENT DESIGNATED AGENCY

This document discloses that a real estate licensee may potentially act as a disclosed dual agent who represents more than one party to the transaction. And it seeks the clients consent for the real estate agent to act as a disclosed dual agent.

What Customers Need to Know When Working With Real Estate Brokers or Licensees:  This document describes the various types of agency relationships that can exist in real estate transactions.  Louisiana real estate agents are required to provide this pamphlet to all clients.

PROPERTY DISCLOSURE DOCUMENT

Effective July 1, 2004, a seller of residential real property must furnish purchasers with a Property Disclosure Document.   The required Property Disclosure Document may be in the form promulgated by the Louisiana Real Estate Commission or in another form that contains substantially the same information.    This Commission form can also be found at www.lrec.state.la.us.
